Certificate Reprint Form
Reprints for the CI-CPT certification and for all certificate programs are available. CI-CPT certifications within two months of expiring will not be reprinted unless renewal requirements are met. Non- CI-CPT certificates (i.e., Cooper certificate programs) no longer expire and may be reprinted at any time. Follow all instructions on the form and keep a copy of the completed form for your records.

Return and Price Policies:
The Cooper Institute will accept returns or exchanges within 30 days from the original purchase. All returns and exchanges must be in the same condition in which they were purchased. You must have the original receipt for items purchased from The Cooper Institute Bookstore onsite.
Non-returnable items include: manuals, publications, testing materials, opened computer software, DVDs and CDs. Shipping charges are not reimbursed.
