Certificate Reprint Form
Effective January 1, 2006, only certificants holding a CI-CPT certification will be required to renew their certification. All other Cooper Institute certificates will become open-ended with no expiration date, and will not require renewal. You may now request a reprint of your certificate holding no expiration. Please fill out this form and send it along with a copy of your original certificate. This form does not apply to the CI-CPT Certification. Make sure you retain the originals and a copy of this form for your records.

Product Refund Policy
Please note before purchasing that returns are not accepted for opened home study courses, CDs, or shrink-wrapped products. Unused clothing items may be returned for product exchange. Software may be returned due to defect for exchange of same software. All items eligible for return will be on an exchange or credit-only basis. Unfortunately, we do not reimburse for shipping costs incurred and returns generally can not be accepted after 30 days from purchase.
